Financial Arrangements

For more extensive or expensive treatment, we are pleased to offer the following payment options:  

Payment Option 1:  We will divide the total fee due from the patient by the number of visits to                  
complete treatment.  This amount is payable each visit.  There is no down payment required.
     

Payment Option 2:  6 Month Interest Free Financing for treatment over $1000 by our office.   We require
1/3 down, due at the start of treatment, with the balance payable in 6 equal monthly installments.  Your
authorization for 6 credit card payments is required.  Payments will be deposited monthly on the day of the
month you request.  


Payment Option 3: For treatment over $1500 we have partnered with the
Dental Fee Plan by Capital One    
No down payment.
Monthly Payment Options are Flexible
·      Payment plans up to 60 months; low monthly payments with a low fixed interest rate.
·      Prepayment anytime without penalty.

Payment Option 5:  You may choose to pay your patient obligation in full for each visit, at the time of the
visit.  Checks credit cards and cash are accepted.

Regarding dental insurance: The Estimated Insurance Payment is based upon the information           
available currently.  Insurance companies do not always pay as expected.  While we will try our best to obtain
the maximum benefit you are entitled to, you are ultimately responsible for any amount your insurance does
not pay.
